Recent Insurance Policy Updates

PA form revised. Added the following question: "Has the patient been diagnosed with progressive neuroendocrine tumors of pancreatic origin (PNET) or with well-differentiated, nonfunctional neuroendocrine tumors (NET) of gastrointestinal (GI), or lung origin which are unresectable, locally advanced or metastatic?"

Removed the following from orthotic devices/services that do not require pre-authorization: L0450, L0454, thoracic-lumbar-sacral orthotic, and L0621-L0630, sacral and lumbar-sacral orthotic.

Walgreens has been removed from the preferred pharmacy network for under 65 plans in favor of CVS Caremark. It remains on the Medicare Advantage network.
